By participating in CMHA Calgary services, you acknowledge and understand that all information provided will be recorded and stored in accordance with CMHA’s data collection and privacy policy. Information collected will only be used for the purpose of providing support to participants, making improvements to service deliveries and, where required, reported in summary for funding purposes.
The information is collected and retained under the authority of the Freedom of Information and Protection of Privacy Act (FOIP), Section 33(c) and Health Information Act (HIA), where applicable.
